EMBED Executive Functioning Therapy Program

EMBED is an executive functioning (EF) therapy program for children aged 3-12 years. EMBED uses evidence-based therapy modalities for improving behavior and executive functioning skills in children, including components of parent-management training, modeling, repetition practice, psychoeducation, contingency management, and monitoring/awareness. EMBED can be delivered in different formats, including individual vs. group therapy and virtual vs. face-to-face therapy. EMBED information and materials are available only for therapists and patients working directly with Dr. Kronenberger. Outpatient Therapy Forms

If you are scheduled for an outpatient evaluation or testing with Dr. Kronenberger, please download, complete, and return the Parent Intake Forms below, before your first visit. Teacher forms can be downloaded, completed, and returned by teachers anytime after the first visit. You will be provided by our clinical staff with a password for downloading the forms.

Learning, Executive, and Attention Functioning Scale (LEAF)

The LEAF is a 55 item parent-, teacher-, or self-report questionnaire that assesses executive functions, related neurocognitive functions, and academic skills in children and adults. Outburst Monitoring Scale (OMS)

The OMS is a behavior checklist measure of specific types of outbursts, including verbal and physical aggression directed toward self, property, and others. Conduct-Hyperactive-Attention Problems-Oppositional Symptoms Scale (CHAOS)

The CHAOS is a 22-item measure of attention problems, hyperactivity-impulsivity, oppositional-defiant behavior, and conduct problems that can be rapidly completed in under 5 minutes by a parent or teacher and scored in under 1 minute with no special software or conversion tables. Pediatric Inpatient Behavior Scale (PIBS)

The PIBS is an observer-report behavior questionnaire for children and adolescents during hospitalization on pediatric or psychiatric units. Quality of Hearing Scale – Revised (QHS-R)

The QHS-R is a 20 item caregiver-report questionnaire that measures functional everyday hearing quality in children and adolescents. A self-report version of the QHS-R for adolescents and adults is also under development. Child Observed Coping Scale (COCS)

The COCS is a 57-item observer-report measure of 19 domains of coping skills in children and adolescents.
