EMBED is an executive functioning (EF) therapy program for children aged 3-12 years. EMBED uses evidence-based therapy modalities for improving behavior and executive functioning skills in children, including components of parent-management training, modeling, repetition practice, psychoeducation, contingency management, and monitoring/awareness. EMBED can be delivered in different formats, including individual vs. group therapy and virtual vs. face-to-face therapy. Referrals to EMBED therapy may be made through the child and adolescent psychiatry services of Riley Children’s Health at 317-944-8162 or contact Dr. Kronenberger for more information.
Pretreatment Questionnaires and Information
After you have been accepted into EMBED Therapy and have an appointment, please complete and return these questionnaires before your first appointment. Instructions for completing and returning the questionnaires are contained in the download. You will need a password to access these forms.
Monitoring Questionnaires
Please complete Monitoring Questionnaires within 24 hours of each session (including the first session). You will need an EMBED group number provided by your therapist.

EMBED Post-Treatment Follow-Up Resources (FUR)(Passwords Provided At Final Treatment Session):
The EMBED therapy sessions are the beginning of a lifelong journey to develop your child’s EF skills. Although most families report improvement during the EMBED intervention, the greatest progress is made by continuing to use EMBED principles after the treatment meetings have ended. Therefore, it is important to make EMBED EF activities a part of your child’s everyday life. Like any life-change, it takes commitment to keep up the activities of EMBED when the treatment visits are over. This is true of many treatments, such as dieting, physical therapy, and exercise plans. It is also true for any skill, such as playing a sport, practicing a musical instrument, or speaking a language. As the saying goes: “Use it or lose it.” The follow-up resource (FUR) modules below are self-directed plans that are designed to continue the progress of the EMBED program long after the formal meetings are over. Download the handouts for each follow-up resource (FUR) plan at the recommended time after treatment. The EMBED FUR Introduction handout will provide more detailed instructions.
